The cash on top of the miles. Air France-KLM Flying Blue states that a carrier-imposed surcharge is charged on an award. Alaska Airlines Atmos Rewards, American Airlines AAdvantage and United MileagePlus make you liable for one if the operating airline levies it, without saying whether it does.
